Has my ceramic coating failed?

Water beading happens at the very top of the coating. Anything sitting on top stops it: road film, traffic grime, hard water deposits. So a coating that has stopped beading is usually still there and still protecting. Coatings do wear out in the end, and most last two to five years. But beading alone does not tell you that.

What kind of repair does it need?

A proper decontamination first. A wash, an iron remover and a clay bar to lift what is sitting on the coating. On most cars the beading comes back and nothing else is needed. If it does not come back after that, the coating on that panel has gone. That panel is polished back and recoated, because a coating will not stick to old coating.

Class of work Decontamination first. A recoat only if that does not bring it back.

If decontamination does not bring the beading back, what is left of the old coating is polished off and a new coat goes on. The preparation is most of the work.

What to do next

Have it decontaminated before you agree to a recoat. Do not accept a strip and reapply quote based on beading alone. If it does not come back, ask the installer about warranty first. If you pay for a recoat, most of the cost should be the preparation.
